Overview

In preparation for an aging society, our hospital has worked to build a system that provides effective medical care by separating the treatment of acute-phase patients requiring intensive care from chronic-phase patients requiring long-term care, thereby ensuring both medical specialization and continuity of care.
Within this framework, our General Medicine department — based on our hospital’s unique approach — focuses on acute-phase care for lifestyle-related diseases and elderly conditions alongside general internal medicine, conducting assessments up to the level of care needs determination and linking patients to continued primary care.
Elderly patients often present with a wide range of conditions. For this reason, our department aims not only to address medical issues but also to comprehensively assess elderly patients and support their independence and quality of life (QOL) in order to provide the most individualized care possible.

General Medicine

After discharge, our home medical care services function as an extension and complement to hospital-based care, with an inpatient system in place for emergencies.
The discharge planning coordinated by our General Medicine department guarantees continuity of medical and nursing care, enabling patients with complex post-discharge needs to make informed decisions.
In addition, physicians and specialist staff are involved throughout the process — including support for long-term care insurance certification, assessment of patient and family needs, self-care capabilities, and living conditions necessary for post-discharge care plan development, as well as care counseling for patients and families — ensuring that continued medical care is provided after discharge.

Dialysis

Our General Medicine department also oversees dialysis care, covering not only internal medicine but also surgical areas, with the goal of providing consistent medical care from diagnosis through treatment for each patient. This approach allows us to select and implement the most appropriate treatment by fully considering each patient’s condition and background factors.

In the dialysis unit, medical management is handled by the General Medicine department, and we pursue the improvement of knowledge and skills through research and training in blood purification therapy. For complications and other issues, we provide safe and comfortable treatment in close collaboration with physicians from each department, working toward the establishment of solid team-based medicine.

  • To support social reintegration, the 2nd dialysis session accepts patients until 18:00.
