What to Bring

Health Insurance Card or Certificate of Eligibility

Your health insurance card is your registered proof of health insurance eligibility.

※ If you do not have it with you, you will be required to pay the full amount yourself. The difference will be refunded when you bring it in later.

Please also bring the following if applicable:

  • Your hospital registration card
  • Public assistance cards such as senior citizen benefit certificates
    (if applicable)

Outpatient Visit Process

    General Information / Returning Patient Reception

    At your first visit each month, please scan your My Number Card at the card reader at the general information desk, or present your certificate of health insurance eligibility.
    Then, please check in at the returning patient reception machine or general information desk.
    If checking in at the returning patient reception machine, please take your ticket and wait in front of your department.

    Examination

    When your name is called, please proceed for your examination.
    If injections or tests are required, please follow the nurse's instructions.
    After the examination, your outpatient guidance card will be returned to you. Please submit it at the cashier.

    Payment

    Please submit your outpatient guidance card at the cashier and wait.
    After approximately 15 minutes, please insert your registration card into the automated payment machine to pay.
    (Cash only. If you do not have your registration card, please pay at the staffed cashier window.)

    If you have been prescribed medication, please submit your prescription to a pharmacy outside the hospital within 4 days (including the day of issue), pay, and receive your medication.

    If you would like your prescription sent to the pharmacy by fax, please inform the new patient reception after payment. (Please provide the pharmacy name and fax number.)

※ Telephone reservations are not available. We recommend making your next appointment before leaving the hospital. Appointments can be made at each department, the general information desk, or the reception.
